The Health Planning Council of Southwest Florida, Inc. (HPCSWF) plans for the health care needs of the residents of seven counties. Early Steps, a component of HPCSWF, is an early intervention program serving children ages 0-3 with developmental delays or disabilities and their families, at home and in their communities in Southwest Florida. Early Steps is focused on parent education and connects families with professionals who teach them how to improve the development of their children through everyday activities.

Child Find works to locate children (age 3-5) who are potentially eligible for services under IDEA and link them with services. The Early Learning Coalition of Sarasota County is the community’s one-stop resource for information about early learning experiences in Sarasota County. They assist families in finding quality and affordable child care and provide resources that enable parents or other caregivers to make informed choices on child care. They also provide information on the School Readiness Program which offers financial assistance to low-income families for early child education and care so families can become financially self-sufficient and their young children can be successful in school in the future. Challenger Baseball is for every child who is unable to participate on a typical Little League team. In the Challenger division, every player gets to bat and play the field in every inning. Children’s Medical Services is responsible for administering the Children with Special Health Care Needs Program in Florida in accordance with Title V of the Social Security Act. The current Title V initiatives of the CMS Managed Care Plan include expanding and promoting the use of a patient centered medical home; providing families and youths with transition education and services necessary to successfully transition into an adult health care environment; and linking appropriate and timely behavioral health services to children. Community Resource Network (CRN) of Florida provides services for individuals of any age, from birth to late in life. CRN is a Medicare and Med-waiver program meaning that those with disabilities won’t pay out of pocket for services. CRN offers personal supports, personal care, homemaker services, respite care, speech and occupational therapies, transportation, and life skills programs.
Florida RX Card
The Florida Rx Card is free for everyone. All residents are eligible to get pharmacy discounts through this program. The program may be used to provide savings on prescriptions not covered by health insurance or Medicare Part D. The program may also be used instead of insurance coverage when it offers greater savings than the copay.

Sarasota Housing Authority Youth Thrive (SHA) provides onsite youth educational and enrichment programs to youth ages 5–17 who reside in public housing. All SHA Youth Thrive programming is on public housing property and free-of-charge to SHA families. The program’s purpose is to engage young people, enrich their lives, inspire them to dream, and empower them to identify and work towards goals that will enable them to break the intergenerational cycle of poverty. SHA Resident Services works with the family by offering education, employment opportunities and assistance where needed. Sarasota Housing Authority provides two portable youth programming/learning centers that include classrooms, computer labs, libraries, kitchenettes, a playroom and a teen center.

The Family Café exists to provide individuals with disabilities and their families with an opportunity for collaboration, advocacy, friendship and empowerment by serving as a facilitator of communication, a space for dialogue, and a source of information. The Annual Family Café brings persons with all types of disabilities together for three days of information, training, and networking each June. The Annual Family Café offers attendees exposure to resources on a grand scale, unprecedented access to policy-making officials, and a chance to network with other individuals with disabilities and their families. There is no registration fee for individuals with disabilities and their family members, and limited financial assistance to cover the cost of hotel accommodation is available.
